About Elder Pathways

Elder Pathways Concierge is the home of Travis Neill, a board certified and geriatric trained Physician Assistant with over 15 years of experience caring for older adults, and Dr. Greg Gahm, a Physician with over 30 years of experience in Geriatrics who has worked with Travis his entire career. For Elder Pathways, Travis provides all of the care while Dr. Gahm serves as a trusted consultant and adviser.

Meet Travis

Travis Neill is a board-certified Physician Assistant and Associate Professor of Geriatrics who has been caring for older adults in Post-Acute and Long-Term care environments, including Assisted and Independent Living, for over 15 years. Early in his career, Travis graduated from the University of Colorado with a Masters in Finance, working in Corporate Finance for eight years before changing career paths and completing a three year Masters in Physician Assistant Studies from the University of Colorado PA program. His passion for learning and teaching has produced opportunities such as lecturing on Geriatric topics at several PA schools in Colorado, authoring articles for various publications, presenting on geriatric topics at conferences and on podcasts, and serving as an expert witness for malpractice cases involving PAs. Additionally, for the past seven years, Travis has worked as an Assistant Medical Director at multiple Nursing facilities in Colorado and was elected to the Colorado Medical Director Association Board in 2018 as the first ever PA on the board in their 30-year history. His passion in caring for older adults has led to his most recent move into concierge medicine where he hopes to improve the standards of care, so our aging population can finish their lives with a focus on health, dignity, and purpose. 

A Colorado native, Travis enjoys skiing, hiking, and mountain biking. He still plays competitive soccer and competes in yearly triathlons. Mainly, he loves spending time with his wife of 17 years and their 13 year old daughter, both of whom give him a run for his money on the ski slopes! Travis also has two dogs, young Bindi and the older and wiser Jack who accompanies him at work as a sweet, empathetic therapy dog. 

Meet Dr. Greg Gahm

Dr. Gahm has over 30 years of experience in Geriatrics and serves as a consultant and adviser for Elder Pathways LLC. He is the Chief Medical Officer for Vivage and a medical director for several of Vivage’s skilled nursing facilities. He attended the University of Colorado for undergraduate studies and medical school, followed by a residency in Internal Medicine in Arizona, a Fellowship in Administrative Medicine and Medical Informatics at Harvard, and a sabbatical in Clinical Preventive Medicine at Stanford.

Dr. Gahm is an Associate Professor of Clinical Medicine for the University Colorado Health Sciences Center and is actively involved in coordinating influenza recognition and treatment efforts in Colorado. In addition, he served as President of the Colorado Medical Director’s Association for four years.

Dr. Gahm and Travis have led a monthly Geriatric Journal Club meeting since 2012 attended by hundreds of Healthcare Providers in Colorado. Dr. Gahm also serves on the Vivage Community Ethics Committee and has done research on Influenza, Antibiotic Stewardship, and COVID-19.

When he is not working, he can be found doing something active outdoors with his wife of 27 years, children or siblings. He also enjoys reading medical journals, and cooking alongside his wife.
