Fees are all inclusive to all services described in the membership.

• Individual Membership Fee: $200 per month or annual payment discount of $2,300

• Couple Membership Fee: $375 per month or annual payment discount of $4,300

Fees are separate from insurance. You keep your insurance for specialists and emergency care.
